PRIVACY NOTICE

This Privacy Notice has been prepared by Özel Kıztaşı Polikliniği (the "Institution"), acting as data controller pursuant to Law No. 6698 on the Protection of Personal Data (the "KVKK "), to inform you about the processing of your personal data through the Contact Form.

As part of the Form, we process your full name, telephone number, email address, preferred communication language and method, the subject matter of your request, and any information you voluntarily provide in the description field. These data are processed for the purposes of receiving and categorizing your request concerning an appointment, general information, information about a medical department or international patient services; routing the request to the relevant department; responding to your request; contacting you through your preferred communication channel and in your preferred language; managing patient relations and communication processes; and retaining communication records.

Your personal data are collected electronically through the form available on our website by automated or partially automated means. Data relating to requests for general information and communication are processed on the legal ground that such processing is necessary for the legitimate interests pursued by the Institution in responding to requests and managing communication processes, provided that your fundamental rights and freedoms are not adversely affected. Where you request an appointment, such data are additionally processed on the legal ground that processing is necessary for the personal data of the parties to a contract, provided that it is directly related to the establishment or performance of that contract. In the event of a dispute or a request from a competent authority, your data may also be processed for the purposes of complying with legal obligations and establishing, exercising or protecting a right.

Where necessary for, and limited to, the achievement of the purposes specified above, your personal data may be transferred, in accordance with the conditions set out in Article 8 of the KVKK, to competent public institutions and authorities, legally authorized private persons, and suppliers from which the Institution receives IT infrastructure, hosting or call center services. Your data will be retained for the period prescribed by applicable legislation or required by the relevant processing purpose; upon expiry of such periods, they will be erased, destroyed or anonymized in accordance with applicable legislation.

If, in connection with your contact request, you provide information about your health status, existing symptoms or the medical department about which you wish to obtain information, such information may be processed as health data constituting a special category of personal data. Pursuant to Article 6(3)(e) of the KVKK, such health data will be processed, to the extent necessary, by persons under an obligation of secrecy or by authorized healthcare institutions for the purposes of providing medical diagnosis, treatment and care services and planning and managing healthcare services. Nevertheless, please do not submit test or examination results, medical reports, prescriptions, medical images or detailed health information through the Form.
