PATIENT RIGHTS AND RESPONSIBILITIES

At Özel Kıztaşı Polikliniği , we recognize respect for patients' rights, patient safety, privacy, and patient satisfaction as among our fundamental principles in the provision of healthcare services.

All patients who apply to our Polyclinic have the rights set out below, within the framework of applicable legislation, the requirements of healthcare services, and our institutional practices.

While patients' rights safeguard fundamental rights during the provision of healthcare services, including the rights to receive information, make decisions, privacy, safety, and respectful treatment, patient responsibilities are intended to contribute to the safe and effective delivery of healthcare services.

PATIENT RIGHTS

1. Right to Access Healthcare Services

Every patient has the right to access healthcare services in a manner respectful of human dignity and personality rights, without discrimination on the grounds of language, religion, race, sex, denomination, cultural differences, or any other basis.

Patients have the right to receive healthcare services under safe, high-quality, and appropriate conditions.

2. Right to Information

Patients have the right to receive clear and comprehensible information regarding their health condition; the examinations, tests, and treatment methods planned for them; and the purposes, expected outcomes, potential risks, and alternatives thereof.

Patients also have the right to be informed of the identities, duties, and professional titles of the physicians and other healthcare professionals providing services to them.

3. Right to Accept, Refuse, or Modify Treatment

Without prejudice to the exceptions prescribed by applicable legislation, patients have the right to decide upon, accept, or refuse the medical interventions proposed to them.

The patient shall be informed of the potential health consequences of refusing or discontinuing treatment.

4. Right to Privacy and Confidentiality

Information concerning patients' health status, medical assessments, examination and treatment processes, and personal data shall be protected in accordance with the principles of confidentiality and privacy.

Patients have the right to request that examinations and treatment be carried out under conditions that respect their privacy.

5. Right to Know the Physician and Healthcare Personnel

Patients have the right to be informed of the identities, duties, and professional titles of the physicians and other healthcare personnel providing services to them.

Patients' requests concerning their right to choose a physician and healthcare provider shall be assessed within the framework of applicable legislation and the nature of the healthcare service.

6. Right to Obtain a Second Opinion

Patients may request an opinion from another physician regarding their health condition and the diagnostic or treatment methods planned for them.

7. Right to Be Treated with Respect

Patients have the right to receive healthcare services in a courteous, considerate, and non-discriminatory environment in which human dignity, personality rights, and the privacy of private life are respected.

8. Right to Safety

Patients have the right to remain in a safe environment throughout the period in which they receive healthcare services.

Kıztaşı Polyclinic takes the necessary measures and carries out the requisite activities to ensure the safety of patients and staff and to provide safe healthcare services.

9. Right to Respect for Religious and Cultural Values

Patients' religious and cultural values shall be respected. Requests by patients to observe their religious obligations shall be respected, provided that such requests do not conflict with the requirements of healthcare services or the provisions of applicable legislation.

10. Right to Express Opinions and Submit Suggestions and Complaints

Patients and their relatives have the right to submit to Özel Kıztaşı Polikliniği their opinions, suggestions, requests, and complaints concerning the healthcare services received and to request that their submissions be reviewed.

Submissions shall be reviewed in accordance with applicable legislation and the Polyclinic's internal procedures.

11. Right to Information on Healthcare Fees

Patients have the right, to the extent reasonably possible, to receive information on the fees for healthcare services before such services are provided.

An invoice or proof of payment may be requested for the services provided, in accordance with applicable legislation.

PATIENT RESPONSIBILITIES

For patients' rights to be exercised effectively and healthcare services to be provided safely and in an orderly manner, patients are expected to comply with the responsibilities set out below.

1. Responsibility to Provide Accurate and Complete Information

Patients are responsible for accurately and completely disclosing to healthcare professionals their health condition, medical history, medications, known allergies, and any other information material to the provision of healthcare services.

2. Responsibility to Follow Recommendations

To ensure the safe and effective delivery of healthcare services, patients are responsible for following healthcare professionals' recommendations concerning the examination, testing, treatment, and follow-up processes communicated to them.

3. Responsibility in the Event of Refusal of Treatment

If patients refuse a proposed examination, test, or treatment, they are responsible for taking into consideration the information provided regarding the potential consequences of that decision for their health.

4. Responsibility to Comply with Polyclinic Rules

Patients are responsible for complying with the rules established by Özel Kıztaşı Polikliniği concerning appointments, examinations, payments, security, hygiene, and the orderly delivery of healthcare services.

5. Responsibility to Respect Other Patients and Staff

Patients and their relatives are responsible for respecting the rights of other patients, patients' relatives, and staff.

Threats, insults, violence, loud speech, and similar conduct that may impede the provision of healthcare services must be avoided.

6. Responsibility to Comply with Infection-Control Measures

Patients and their relatives are responsible for complying with the hygiene, mask-wearing, isolation, and other infection-control measures communicated to them in order to prevent the spread of communicable diseases.

7. Responsibility Regarding Appointments

Patients are responsible for arriving for their appointments as punctually as possible and for notifying Özel Kıztaşı Polikliniği in advance if they are unable to attend.

8. Responsibility for Fees and Payments

Patients are responsible for paying the fees arising in connection with the healthcare services received, in accordance with applicable legislation and the terms communicated to them.

9. Responsibility to Exercise Due Care in Respect of Property Belonging to the Institution and Healthcare Professionals

Patients and their relatives are required to use the equipment, devices, fixtures, and other materials belonging to Özel Kıztaşı Polikliniği with due care and shall be liable, in accordance with applicable legislation, for any damage caused intentionally.
